TERMS OF ENGAGEMENT BETWEEN THE REGISTERED NUTRITIONAL THERAPY PRACTITIONER (RNTP) AND CLIENT
PART 1
The Nutritional Therapy Descriptor
Atsike Nutrition Limited practice in accordance with the British Association of Applied Nutrition & Nutritional Therapy’s (BANT) ethical framework.
Nutritional therapy is the application of nutrition and lifestyle medicine sciences in the promotion of health, peak performance and individual care. Registered Nutritional Therapy Practitioners (also called Nutritional Therapists) assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows me to work with individuals to address nutritional imbalance and help support the body towards maintaining health.
Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a ‘one size fits all’ approach.
We will never recommend nutritional therapy as a replacement for medical advice and always refer any client with ‘red flag’ signs or symptoms to their medical professional. We will also frequently work alongside medical professionals and will communicate with other healthcare professionals involved in a client’s care to explain any nutritional therapy programme that has been provided.
We request that the Client notes the following:
• The degree of benefit obtainable from nutritional therapy may vary between clients with similar health problems and following a similar nutritional therapy programme.
• Nutrition and lifestyle recommendations will be tailored to support health conditions and/or health concerns identified and agreed between both parties.
• RNTPs are not permitted to diagnose, or claim to treat, medical conditions.
• RNTPs’ recommendations are not a substitute for professional medical advice and/or treatment.
• Your RNTP may recommend food supplements and/or functional testing as part of your nutritional therapy programme and may receive a commission on these products or services.
• Standards of professional practice in nutritional therapy are governed by the Complementary and Natural Healthcare Council (CNHC) Code of Conduct, Ethics and Performance.
• This document only covers the practice of nutritional therapy within this consultation, and we will make it clear if we intend to provide services that are outside the scope of nutritional therapy practice.
The Client understands and agrees to the following:
• I am responsible for contacting my GP about any health concerns.
• If I am receiving treatment from my GP, or any other medical provider, I should tell her about any nutritional recommendations provided by my RNTP. This is necessary because of any possible reaction between medication and the nutritional programme.
• It is important that I tell my RNTP about any medical diagnosis, medication, herbal medicine or food supplements I am taking as this may affect the nutritional programme.
• If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my RNTP promptly for clarification.
• I understand that the recommendations are personal to me and may not be appropriate for others.
• I must contact my RNTP should I wish to continue any specified supplement programme for longer than the originally agreed period, to avoid any potential adverse reactions.
• Recording consultations using any form of electronic media is not allowed without the written permission of both me and my RNTP.
• It is my responsibility to ensure I have a working internet connection for online appointments. If I am unable to connect as scheduled, the appointment is considered a ‘no show’ and charged in full.
Confidentiality and Data Protection
The confidentiality of our clients is treated seriously at Atsike Nutrition. There are however, some limits and exceptions to confidentiality.
The Registered Nutritional Therapy Practitioner will keep your personal information confidential and secure following the UK GDPR guidelines for the practice. The RNTP will not share your information with third parties without your consent. However, if the RNTP believes there is a risk of significant harm to yourself or another person, the RNTP may pass the information onto an appropriate authority using the legal basis of vital interest.
At your initial consultation you will be asked to provide your GP’s contact details, as someone we may contact. Atsike Nutrition Limited keeps a record of your name, contact details and dates and times of appointments. Your Nutritional Therapist is required to keep brief notes about the focus of your work, which are kept securely. Any reports produced by Atsike Nutrition Limited to evaluate the service contain anonymised data and will not contain any identifying information of clients. Complaints & Concerns
If have any concerns or problems with the service provided, which you have not been able to resolve with your Nutritional Therapist, you may raise this by emailing clinic@atsikenutrition.com who will support you in finding a resolution.
Working with Multiple Consultants
Your Nutritional Therapist may suggest you see other wellness professionals to support your goals. Information about your symptoms and condition will be discussed between team members in a secure area so that we can provide you with a coordinated treatment plan. Your Nutritional Therapist will ask for your informed consent to share information with the team and ask if there is anything that you want to remain confidential.
PART 2
In your first session we will discuss and agree the support you require from your Nutritional Therapy, working together we will review the progress and aims as we go along. At this time you are required to sign the bottom of the page to agree to our terms and conditions. Please speak with us if there is anything you would like clarified.
Payment Terms and Cancellation Policy
(aka the small print)
Fees are agreed in advance of the appointment and payment is required in advance of all appointments.
Package fees are paid in full in advance of the first appointment. A debit or credit card number will be requested at the time of booking when booking online. If your therapist creates the booking/package for you, an invoice will be sent to you that is required to be paid in advance.
Instalment payment plans are available for the 3 month and 6 month packages only.
Once payment has been made, it is advisable to book the first batch of package appointments in advance.
Packages are designed to be completed within 6, 12 weeks or 6 months period.
If this is not possible (special circumstances may apply), the programme may be extended to a maximum of 9, 16 weeks or 8 months respectively according to the package taken.
Any outstanding sessions not taken beyond this time frame specified above may be forfeited.
Cancellations/rebooking:
If you do need to cancel an appointment at short notice, please let the registered nutritional therapy practitioner know at the earliest convenience so we can reschedule your appointment. You can do this by emailing: clinic@atsikenutrition.com.
Appointments missed or cancelled with less than 48 hours’ notice will incur a 100% cancellation charge.
Non-attendance of an appointment (no-shows) will also be charged in full.
It is your responsibility to ensure you have a working internet connection for online appointments. If you are unable to connect as scheduled, the appointment is considered a ‘no show’ and charged in full. There are no additional charges for phone consultations to a UK landline or mobile number.
A 15 minute wait time frame will be applied to all sessions. Once 15 minutes of your appointment has elapsed, and you haven’t arrived at your session, the session will be ended and will be classed a ‘no-show’. If you no-show for your appointment you will be sent another invite to book a new session which will be deducted from your package. Refunds and reschedule options will not be offered for no-shows.
